This happened to me recently. I had made myself a pair of shoes using the bare feet as a base. I didn't really know what I was doing so it's all trial and error. In my case I had categorised the shoes accidently as swim wear and they showed up when I was looking at hair. I didn't know what was causing the problem until I brought the camera back and realised the sim had my shoes on. When I made the shoes I didn't know to delete all the texture except the shoe part. Once I did that everything went back to normal. I think maybe something your sim is wearing still has the full texture applied.

so now its not there im still having the same issue with he skin and the hair things i have no idea what it is that is doing it i know it is a cc as it is fine without cc in game , but i dont even know where to start , i have sims studio installed and im looking through all my cc but i cannot find what it is that is causing this issue :(

The best thing to do is the 50/50 method until you find the file. Remove all your CAS cc, then add half back in. If it works ok try the other half. One of those halves will be holding the bad file. You may have more than one so check each half separately. Repeat the process, narrowing it down until you isolate the file(s).
This is clearly a CAS cc item. They only way you can get extra underwear or extra brows on your sim is if creator used a basic body image as a guide and saved the project with this picture on background. So whenever you use CC like this you basically see a skin overlay. It has nothing to category of the clothes but only with faulty texture. I recommend using Tray Importer to track down this kind of content.
Please don't DM me here or elsewhere asking for assistance with Ninjaripper or CC. I don't work in Ninjaripper anymore. Please post your questions in Creator Help thread so everyone can chime in and help solving the problem
