I'd first check your system for updates. Grab the most recent video card drivers and I know it seems silly, but I'd make sure windows is updated, virus scanner, and that you've used the optimizer and disk cleanup. It can sometimes help.
Next thing to do is back up your save games folder and your tray folder. After that, I'd delete the stuff in the sims 4 folder that has the word "cache" on it and make sure the mods folder is clear. (but it seems like you did that.) Make sure there's no reshade or other like applications in there too. We are trying to figure out if it's the game or something else. Next make sure the game is up to date and then do a repair/verify of the game. Try a brand new save to see if your save game is corrupt. If the save game is corrupt then you just need to do a new save and hopefully you have your sims backed up in your tray folder.
If none of that helps, then try a full reinstall of your game, try the old save and a new save and see if you still have the issues. If there's still an issue after that, it might be best to report to EA for further assistance because you'll have done pretty much everything they would initially ask and they can do some more in depth troubleshooting.
Good luck
