Ah ok...what would I need to do that? I have seen some wigs on Mod The Sims (well just one set for females) and wanted to do some variations and some for males

Well I can't find any mannequin wigs on MTS so I'm not sure how they were done. What I would suggest though would be that you clone the hair you want as a wig and extract all of the meshes and textures using the options in Sims 4 Studio. Then clone a hat that you will turn into a wig. Replace all of the textures in the hat package with the ones you extracted from the hair package. Open the extracted hair meshes in blender, one at a time, and delete the groups that are not a full hair mesh. There will be three groups in each hair. One is the full hairstyle and the other two have the top cut off at different angles. Only keep the full one. Import your edited meshes into your new hat package. Set your flags and categories and then test it in game. There are various tutorials on this forum covering various stages of a project like this but nothing that will 100% take you step by step on how to do the exact same thing you are wanting to do. So you may have to read a few and figure out how it applies to your project. The general basics should be the same though.

The mannequin heads are constructed like hair, with the same regions and three meshes - the head with mannequin hair and two bald heads that I guess are the equivalent of hat cuts. IMO by adding an item that also has all the hair regions you create a conflict and the game removes the head.
You could try editing the Region Map to remove all but regions HairHatC and HairHatA and change the region layer to 20001 (like a hat), or just start with a cloned hat as MenaceMan explained above.

ok...sorry I haven't been well so I haven't been here for awhile...so I need blender and do I make a stand alone CAS or create 3d mesh in sims 4 studio? Can I use any hat?

Well Blender is like Double Dutch to me and I can't even see where to open files or what to do to get the stuff from Sims 4 Creator into it...and as for the wigs on The Sims Resource...just type in wigs on the search bar and it pops up...it is just one female hair...and I have tried asking the creator about it but I don't think they are on there anymore...maybe someone here who knows what they are doing can make them...as I am no way going to be able to fathom out blender...it looks way too complicated

You would need to use the Create 3D Mesh option in studio to start your project. You could choose any hat as a starting point but I would make sure to use a hat from the base game.
Blender looks like a scary beast to begin with and doesn't have the most user-friendly user interface but it is fairly straight forward once you understand the basics. Unfortunately, how to use blender in general isn't something that is taught on this forum and it is recommended that you watch tutorials on youtube to learn the basics.
To open a mesh from the game you would export it from the Mesh tab in studio which creates a .blend file in the location you choose. If you then double click on the file it should open in blender automatically.

Thank you...I will try again when I am a bit more with it....pretty poorly at the moment and my brain is scrambled...thanks for all the info so far. If I get stuck I will post on this thread here again.
