I was wondering if anyone could explain the basics of creating a skin overlay in beginning friendly terms. I know that it's basically a normal map that goes over the EA mesh, but really nothing as to how to make it. Is this something that can be done a 3d software program, and than exported as a 2d texture, or do I need to start in 2d? 3d would be easier for me as I'm a decent sculptor but hopeless drawer.

This is not a normal map. This is a grayscale map that is painted by hand. You can extract any custom skin overlay to see how it works. I highly doubt that it can be done in 3D since it is not a combination of normal map, specular and shadow map, it is a pure diffuse map.
