The arms are inside of the body and I've had this problem before but never have figured out how to solve it. Did I append the torso and arm mesh to the wrong mesh/bones? Thank you!

In adition of what ciablue said, there was a version of Harmony that has a bug about this and caused this issue. If for some reason you are using Harmony and haven't updated that might be problem.
