Why is it different from what you see in Substance Painter?

Hi. We need to see the blend and package files. It is possible that the uv maps are in the wrong order or have different names

It looks like your materials haven't exported correctly. Try and export them with these settings:

Just in case you did not, you should bake everything (paint, lightening, etc) into texture in substance painter and export that texture, not the materials or the mesh, just the texture. The Sims 4 has its own materials, lights and rendering engine, it cannot render materials from external software
