Like, have a sim model open in Blender or something and just...see the changes you're making?
Or even paint directly onto the model?

Hi. You can paint in Blender in Texture mode, you can also paint on the model in photoshop ( so long it supports 3d-related features). There are in-depth tutorials on both topics on youtube

If you want to see how a texture looks on your mesh as you're working with it, select your mesh, edit mode, A to select everything--now, your UV islands are showing. You can also enable Keep UV and Edit Mode Mesh Selection in Sync and the UV islands will show without your mesh selected. Press A to select all islands (they'll turn orange). Image > Open Image, open your texture. Because you selected the islands first the texture will be assigned to your mesh. Go into texture mode. Press N to open the side panel. Change the shading to Multitexture. Now you can see how the texture changes when you edit or move any UV island.
