You can set the autonomy, only pets, toddlers, and babies require any extra attention. If you skip those, then they can pretty much run on their own.
DLCs are not required. They can add to the experience and provide you with many new items, supernatural species types and things to do. There are many mods out there that can change the experience. You will need to watch which ones you download and make sure they are compatible with your purchases. Some people use DLC items as bases for mods, so if you do not have the DLC you won't get anything out of a mod that uses it as a base. However there are many who make the items into BGC items.
Alternatively you could just use it as a building game. You technically never have to play the sims... you could just build mode and never go into game mode. I use it as a set design for story telling. I haven't played gameplay in months tbh.
I bought all of it for the items tbh... I don't think I bought a single pack for anything having to do with gameplay. BUT... I've been collecting for years, so making a huge purchase, I would only get those things that interest you and then wait for a sale for anything else.
