Hello, the problem is that when you edit the skirt image(called diffuse) you're editing the texture that displays on a mesh. The mesh is basically the geometric structure where the texture will show up. Here you edited the texture to look like there was a hole, but the actual geometry doesn't have that so it shows the leg skin on it. You will have to edit the mesh if you want to cut something there. There´re some tutorials in the forum about editing meshes, I'd recommend you to read them. Go to the full index at the CAS tutorials. It will help you find them easier.

Hi oreocookie2016, the issue you're running into is that Sims 4 doesn't have a shader for clothes that allows true alpha. The alpha it has will put skin texture wherever a normal alpha puts transparency. As mathcope said, if you use an alpha image on your diffuse texture it will show up with skin in the places where you were expecting transparency because of this deficiency in the game's shader system.
The only shader for CAS items that allows alpha is the glass shader that EA uses for the alpha on glasses accessories. I don't think it would be an easy task to get the effect you're looking for using that shader. The most direct way to get a skirt like the one you have in your picture to show in the game would be to mesh the detail rather than count on an alpha texture to do it.
A second issue is that Sims 4 garments are not being placed on an actual Sim model in the game. A Sims 4 garment includes legs, arms, neck, and etc. as a part of the mesh . If you want leg to show the leg that's showing needs to be part of the mesh.

You could make it a sheer area instead. I would make the part to be transparent as its own vertex area. Separate it from the rest of the mesh. So then you have 2 meshes. Then on the texture, down at the bottom right in the extra space, put a block texture of a color, say lighter pink, And make it transparent. Then map the little transparent part of your skirt to the transparent pink part on your texture. You could even make it really light. The point is moving the texture off the legs onto the side of the map, give it a texture to actually see instead of the leg texture. Don't forget for this you need to use a 2 object skirt because you will need to set the transparent part as a glass.

If you go that route it might be helpful to look at Untraditionalnerd's tutorial on how to include transparent parts in a skirt. That tutorial is HERE.
