A pose (for a sim) works with predefined rigs (made by EA) that define how the object moves and deforms. By changing the rigs in Blender, you can change the sims pose (see the tutorial here). However, I doubt that your object is rigged, am I correct? The only way to then change the pose is to either rig the doll yourself and then pose it (if you plan on using the doll in multiple different poses this would be the way to go, YouTube has some good beginner's tutorials) or change the mesh by hand (which would be a little tedious but doable and probably faster if you only want one pose).

This is not a common topic and it's not specific to ts4. If there's no one right now to help you out you either need to wait or look on a specialized blender forum. Search in youtube for tutorials in the mean time, since i haven't seen anyone rigged any item around.
