Do you have s4studio cas tools in the version of blender you're saving the file?
If no, make sure blender path leads to the .exe file in the installation folder, restart blender. If it does and you still do not have s4studio cas tools, install blender 2.76b instead and change the path to it.
That said, this is most likely not the cause of your problem.
You must never transform the mesh in object mode, s4s does not import that information and applying transforms to deltas does not help.
Everything must be done in edit mode.
To not have to re-do everything, do the following:
- in object mode, Object - Snap - Cursor to center;
- Add - Plane (do not move or scale it);
- vertex paint the headband with 007f00;
- clone earrings - not a hat - and import the blend there, re-categorize the item as a hat

- select s4studio_mesh_1, shift-select the plane (in that order), join with ctrl j, in edit mode select the plane and delete it;
- set type to GEOM and cut to 0000;
- you need to rename UVmap to uv_0 and create a second one called uv_1. Then you need to make the head selectable and transfer uv_1 from it;
- you need to rename UVmap to uv_0 and create a second one called uv_1. Then you need to make the head selectable and transfer uv_1 from it;
- clone earrings - not a hat - and import the blend there, re-categorize the item as a hat
