Hopefully this make sense every time I go into game or s4s my object is always facing the side of the item. I always have to rotate it and how am I able to change the direction? ibb.co/sydkKzL

Is it a deco object without any other functions? If so, rotate it in Blender so it faces the proper direction.
There's a "front view" in Blender, S4S and (a relative one) ingame. For most objects, this is usually the side that points towards you when you place the object or when you view it. If the original cloned object has a particular orientation in relation to the Blender/S4S view, you'll usually want to keep the same orientation for the object you're making, too.

yes it is a deco item and the item that I am using is the pencil cup when I place it in blender how do I know what side is which

In Blender, if the "move" arrows are set to "global" in the transformation orientation, you'll want the green line/arrow (Y-coordinate) to point toward the camera, the red line/arrow to go across the screen horizontally (X-coordinate), and the blue line/arrow to go vertically (Z-coordinate). The X and Y coordinates show in the grid system, but it is a bit easier to see it with the arrows.
You can also go to "view --> front" to set the camera to front view, then move the camera a little bit. This would let you see a bit easier which way the camera is placed in front view.
The front view is what will be towards you when you place the object ingame.
Just for the sake of testing things out, it's a bit easier to understand if you import an item that has a clear front/left/right/back (a chair, dresser, anything like that), so you can look at the orientation and get a better understanding of how Blender works in relation to S4S. A lot of the deco objects are meant to look decent from several sides, so it's harder to see the orientation.
