Hi. We would need to see the blend and package files of the item to tell the exact reason, but it is related to rigging. The gray outfit is affected by bones the original maxis item you started the package from was not. You will see this issue in cas or in live mode if you switch to the highest graphics settings

The item cannot be affected by all the bones in the rig, there's too many of them. You need to either delete some of them or separate the head. You also must not start the package from the nude bottom, it is affected by different bones.
Separate the head and use it as a hat. Clone ymBody_EF10Noctis and import the rest of the body there

in s4s main menu click on CAS, paste ymBody_EF10Noctis in the search bar

I tried to separate the head and import the rest of the body into ymBody_EF10Noctis and the result has not changed. I don’t know if it worked for you.

Decimate removes vertices and faces from meshes. It doesn't touch the bones.

When you split the mesh, you need to remove unused vertex group and restart the package. There are too many of them in the list, so they are causing problems Download this addon ( type in 0, press I want this, enter your e-mail, install this file for the appropriate version of blender)
