Hi there! So I'm not sure what the previous problem was, or what all you have done, but that to me looks like it could be the vertex paint, for the skirt riding up. Check this tutorial. As for the spike going up through her head, that is probably a joint issue, check this tutorial I would just copy the weight from a nearby vertex. I hope that helps!

Ok, no worries, first I would change the mode to "Vertex Paint," instead of "Edit Mode," then see if the colors on the dress are even, and if they are not (which is what I suspect) then paint them the same color I numbered the steps. For step 3, click that little white box, then a new little menu will pop up there, click on the color dropper on it, and then use the dropper to select the same color on the mesh. It should then change to a tool for painting it. Color it so the messed up side matches the good side.

As for the spike going through her head, find the problem vertex, then do this: 1. Select the problem vertex 2. Hold shift and click another vertex near it 3. Click "copy," this will copy the weight from the good vertex to the problem one. If that doesnt work, or you need anymore help, let me know!

For the shadow at the bottom end of your mesh, please check this tutorial. I highly suggest you having "smooth shade" in object mode to see the changes right away, and limit selection unticked. As shown here.

You have to select all the legs in edit mode, and clic the remove doubles.
