I have tried within the last several days to import dresses (mesh) that I have created in Marvelous Designer and they have all looked like this upon import into Blender 2.78. Can someone please tell me why this is happening?
As a side note I know this picture shows the mesh imported with the sims 4 avatar but I did try to import it both ways (with and without the avatar) I got the same result.

How are you importing it into Blender? The way I do it is I open a nude female top then import the mesh obj top.

It's very weird. Somehow the axises of your MD export do not match the ones in blender. Changing Forword to '-Y Forward' and Up to 'Z Up' when you import will fix it.  |

I found the cause. Can you check your MD export dialog again? In Axis Conversion section (you have it collapsed in your screenshot above), it should look like this:  You don't need extra settings in blender afterwards. Your setting must be messed up somehow. |
