You should verify that ALL bones are assigned correctly. Take a look at THIS tutorial if you aren't sure.

ALL bones are assigned correctly. but when I import for S4S, the mesh loses the weight

Check if you have any bones with empty weights. When you transfer weights with 2.76 each vertex get all bones that present in reference blend. Some bones get assigned proper weight and other will show 0.000, this will cause glitches. If it's not the case please share .blend and .package, otherwise we can keep guessing forever.

I don't use 2.76 but that would explain why I see exactly what you describe on every single vertex on the mesh. (And would explain another weight issue on another thread) Because appart from the thigh twist which is not assigned, every bones have correct weight. But all of them have all the bones attached with 0 value when not used. So what would be the solution ? delete all groups and do the transfer with blender 2.70?

One solution is to do all weight transfers in 2.70 to avoid this issue. Another solution is to clean all empty weights after transfer. Not sure if there is a way to prevent getting empty bones when doing weight transfer in versions 2.76 and above. Merman Simmer as I suspected you have empty weights on every vertex. You can check it either in weight mode vertex selection or edit mode vertex selection. If you click on any vertex you will see list of bones and weights. To remove all empty weights from all vertices you need to go either to weight paint mode vertex selection or edit mode vertex selection. In Edit Mode on a Tool tab scroll down and roll out Weight Tools and press Clean. Make sure to select Subset: All Groups and then click "Clean" again. Click any vertex to make sure that all empty bones gone.
