You have it set as an eye color in S4S. If you want it to appear with a thumbnail, you'll want to have it set as something like face paint.

There is a way around it... I downloaded some Non-Default eyes recently where the thumbnail actually shows up in CAS. It may not be something Sims4Studio can do... I'm hoping the creator will enlighten me.
I've reached out to them... and I will report back if they let me know.

You know what... I didn't even need to wait. I figured out what was going on... You will have to manually create images of the eye colors and import them into the package. The game won't automatically created them for you. And they must be 32x32 pixels in size or they will be broken in the game. Its going to take a while to add thumbnails to my existing pile of CC eyeballs but it will be worth it. All the solid color squares make me nuts!
