I have been working on two separate projects and have encountered this same issue. Basically, I take an EA mesh, edit it in some way or combine it with another mesh, do the weight transfer, fix up the UV and it's great! That is, until I put it in the game and move the weight sliders and it distorts the mesh. Example 1 (.blend file) is a choker I made. Originally, the buckle was just a flat texture, but I decided I wanted to take the buckle from the back of the basegame choker and put it on the front. It's fine until you increase the overall weight slider of the sim and then the buckle slips into the neck. Example 2 (.blend file) is a frankenmesh of the basegame long sleeve shirt and the ripped zombie outfit from Spooky Stuff. I transferred the weights from a quarter length tee from the basegame and the section where I replaced the bottom of the shirt with the rips caused the shirt to split apart. Now, I'll be honest, I don't know if these two things are caused by the same issue and I'm sure they have something to do with weights or the uv_1 but I just can't figure it out. I tried looking to see if people have experienced this issue before but those threads didn't help me to understand what was wrong and how I should go about fixing it.

Do you also have the blend files you used for these items? I know they can be extracted from the packages but if there is a problem with the originals Studio might not export them the same so it is best to provide them as well.
I'm not sure about the choker but it looks like the problem with the shirt is that you haven't connected the UV_1 bits properly. You need to snap the vertices from your added part together to the vertices of the original part so that they match and continue the shape. You have just sort of layed your additional bits in sort of the right area. Some bits overlap and some bits have gaps in them.

menaceman44 Thank you for the reply! I updated the two download links on the top post so that they lead to the actual blend file instead of the package file. Now, to what you suggested with the shirt, I did snap the vertices from the added bits to the shirt, which is why it's confusing that it's still doing that.

I've just looked at your shared blend and the same issue is still there. You have snapped the UV_0 together correctly but not the UV_1. That one still needs fixing.

menaceman44 I snapped the UV_1 together and it finally worked. Well, I had to do it a couple of times because the game needed it to be exactly placed on top of each other. It's working fine now, thanks! I still not entirely sure what to do about the choker though, but I'll try messing with that UV_1, too
