Trying to edit the bear doctor kit

I have been struggling trying to figure out how to edit the kids doctor kit with the bear and I changed the mess so it's this little frog doll. When it's sitting on the floor everything looks great but when the kid goes to pick up the toy, it turns back into the bear. How can I change it so the bear stays the frog through the whole animation?
