Is this something that's possible? I used one in Sims2 and I feel like it should be possible since the ghosts are translucent.

Hi. TS2 has alpha hair by default, TS4 does not, so the shader used for the skin does not support transparency. You would need to change the shader of each body part, including the head. Presumably, a ghost-related tuning file forces the change of the shader on the whole sim. I think that you could create a trait that forces this change on the sim.
But to answer your question, no, it is not possible to make a see-through skin through skin(textures) alone
