Hey I am very new to this so I could just be doing something stupid, I tried making a custom 3d mesh and it appears to be fine in sims 4 studio and in blender but when I open it up in game it looks like the model was but into a real life blender. I've messing with it for hours, restarted a couple of times and nothing seems to work. Well, we would need to see the blend file, but i assume that you did not remove some of the original bones from the mesh - select the head, delete all vertex groups; - create a new one with + button, double-click on it to rename to b__Head__ ; - switch to edit mode, select everything with A, assign to this bone; - clone a new package, do not re-use the old one, import the blend there

Thank you that helped a little bit, now the head isn't on the body. I added the blender file and a new image of the problem.

it must be assigned to the head bone only, delete b__R_ThighTwist__ If you want the mesh to morph with sliders, you need to retransfer uv_1, it is very torn. If you do not need it to, delete both entries in color attributes, create a new one, name it color0, set up like in the pic Also, your mesh must not contain the head underneath, accessories do not contain body parts Enable sync, press B, click and drag to select uvs of the head, in 3d view press Delete, choose Faces
