Modding Request - Name Necklace

I've tried and tried to make this myself, but my skills aren't up to something so small and delicate. If someone could make a simple name necklace for me, I'd be so greateful, I just can't figure out how to do it. I'm just looking for something pretty and simple in gold, with a the chain coming in at either end.
