It is rigged incorrectly. The sum of all weights must be no more or less than 1 ( 100% ), right now it is 2. b__R_LoLip__ must not affect the mesh at all, so delete it with - button in Vertex group field. There are other issues: - the ears you're seeing in the scene do not match the ones sims have in-game. Clone yfHead_Mannequin, export the blend, append in the scene, move your earrings to match this head; - uv_1 should be transferred from yfHead_Mannequin, not maxis earrings, since their placement differs. When transferring it, choose Nearest face interpolated. If you'll find them stretching in-game, you may need to weld uvs of each earring; - not a big deal, but the last uv map can be deleted

The head you see in blender and in s4s is different from the one used in-game, check in cas

Seounds like you removed weights incorrectly. - open your blend file; - with b__R_LoLip__ selected, click on - button to delete it if you havent yet; - in Edit mode select everything with A; - with b__Head__ selected and Weight set to 1, click on Assign; If wont help, share the updated blend file

Ah, not floating away from the body. The ears your sim has are very different from the standard ones, so you need to adjust them to match earrings. Also, uv_1 is still the same. Not like re-doing it would help much, since they cant follow ear morphs precisely, but re-transfer it with Nearest face interpolated from a mannequin. Now you see why there is nothing like this among maxis items
The third preset seems to be the closest
