So I have created a mesh: picture below But it was looking a bit bumpy so I decided to add a subdivision surface modifier to make it smoother however, this created holes in the mesh which I dont want. Is there a way I can get rid of this or another way to make the mesh smoother. Thanks in advance

Try removing doubles and then adding the subdivision surface modifier.

You could also try to recalculate the normals. It might be part of the reason why the holes are present.

hi! I wouldn't recommend using that modifier, since it duplicates the geometry of your mesh making it more high poly than it should. You can use the "smooth" brush on sculpt mode to make the mesh smoother in the places you think it lacks. But previously, you need to remove doubles just like slyd suggested.
